On 28th March 2019 the West Berkshire Dementia Action Alliance are hosting their Understand, Stand Together dementia conference. This is aimed at individuals and organisations that influence or impact the lives of people affected by dementia in West Berkshire. Exploring how we can work more cohesively together, looking at the challenges and opportunities that we face both strategically and as a community around dementia.
In order for the conference to raise awareness of what it is like if you are affected by dementia here in our community, it world be great to gather feedback from as many people as possible who are living with dementia or caring for someone with dementia. A questionnaire was produced to give everyone the opportunity to share their views and experience.
If you have any colleagues, service users, clients, customers, patients, family and friends, please share this survey with them and encourage or support them to complete it. The data will be collated and analysed to provide reporting information which will be shared at the conference and with other stakeholders.
Please send completed questionnaires to:
Sue Butterworth, Public Health and Wellbeing, West Berkshire Council, West Street House, West Street, Newbury, West Berkshire, RG14 1BZ
