To enable people to remain in their own homes and reduce avoidable hospital admissions, Berkshire Healthcare NHS Foundation Trust provide nursing services at home to patients who are unable to get to a doctor or clinic due to their health condition.
We work with patients to enable them to be as independent as possible. Where appropriate we teach them about self-care, for example advising them how to manage their own medications or give injections.
We assess patients at home and create a plan of care for them. We can provide intravenous therapy and other high tech services at home which are unavailable in a GP surgery. We also run specialist leg ulcer clinics in five locations.
You may be referred to our services by your GP or following a stay in hospital.
